Alternatively, send us an email at service@drschoice.com.au with “Sign Up” in the subject and we will forward the documents to you.

 

For sign ups by Practice Managers, please send us an email at service@drschoice.com.au with “Practice Sign Up” in the subject and we will forward the documents to you or contact us on 1800 DRS CHOICE (1800 377 246).

 

Step 1 - Appoint us

Download and sign the Doctors’ Choice Appointment Letter. 

 

You should read our Financial Services Guide (FSG) and Privacy Policy which can be found here.

 

Step 2 - Provide your information

Download, complete and sign the Doctors’ Choice Insurance Information Form which is relevant to you (Specialist, GP, PGY/Doctors in Training, Student/Intern or Practice).

 

Step 3 - Return the documents to us

As we don’t need the originals, you can scan the signed Appointment Letter and Insurance Information Form and email them to us at service@drschoice.com.au. If you prefer, you can fax them back to us on (08) 6323 4668 or mail the originals to us at Suite 19, 23 Mill Point Road, South Perth WA 6151.
